CHILDREN ENTERTAINMENT SPACE „DŽIAUGSMOTEKA”

 Consent

One of the Child’s parents/other legal representative of the Child or an adult authorised by the Child’s parents/other legal representatives to accompany the Child (hereinafter referred to as the „Person Responsible for the Child”), by signing this Consent, confirms that:

  1. Has read and familiarised himself/herself with the Rules of the Children’s Entertainment Space „Džiaugsmoteka” (hereinafter referred to as the „Children’s Entertainment Space”) (hereinafter referred to as the „Rules”), understands and agrees with them. He/she has also read the Rules of the Children’s Entertainment Space to the Minor Child(ren) named in this Consent (hereinafter referred to as „the Child”) whom he/she is admitting to the Children’s Entertainment Space either alone or who will be present in the Children’s Entertainment Space together with the Person Responsible for the Child.
  2. Has read, understood and agreed to the Children’s Entertainment Space Privacy Policy.
  3. Acknowledges that he/she is aware that the Children’s Entertainment Space Rules and the Children’s Entertainment Space Privacy Policy are publicly available in the Children’s Entertainment Space and on the website located at www.dziaugsmoteka.lt (hereinafter referred to as the „Website”).
  4. Undertakes to comply properly with the Rules and to ensure that they are complied with by the Child, and accepts full responsibility for non-compliance with the Rules, including, but not limited to, liability for damage caused by the Child to the Child and to third parties.
  5. The Child agrees that in the event of the Child’s failure to comply with the Rules, the entire liability shall be borne by him/her as the Person Responsible for the Child and understands that„Džiaugsmoteka, UAB“, registered office 305102859, address Karaliaus Mindaugo ave. 49, Kaunas (hereinafter referred to as the „Operator”) shall not be liable for the occurrence of, or indemnification for, any damages arising out of the Child’s reckless, dangerous behaviour or other failure to comply with the Rules.
  6. The Operator has adequately assessed the services, facilities, environment, his/her own and the Child’s abilities and possibilities to visit the Children’s Entertainment Space and understands that the Children’s Entertainment Space contains activities, entertainment and equipment for children of all ages from 1 to 14 years old, and ensures that the Child will use the Children’s Entertainment Space’s activities, entertainment and equipment in an age-appropriate manner for the age of the Child.
  7. A Child admitted to the Children’s Entertainment Space shall not have any medical or other condition which may cause adverse consequences or safety and health risks to him/her and/or third parties in the Children’s Entertainment Space.
  8. Is informed and consents to the collection and processing of the personal data referred to in this consent for the purpose of visiting the Children’s Entertainment Space. The data will not be transferred to third parties and/or used for marketing purposes without the prior consent of the visitor.
